Inital Fees
Admission: $75.00 – (includes all initial lab fees, physical, and assessment)
 
Methadone Patients
Daily: $11.00
Weekly: $70.00
Monthly: $280.00
 
 
Suboxone Patients
Medical Management Fee - $175.00 per month (includes physician follow-up, one drug screen, and individual & group counseling services)
Medication - Medication charges will be as follows (NOTE: If you have health insurance and would like to purcharse your medication at a pharmacy these charges will NOT apply to you):
                      
                       Average dose level at 16 mg - $10.00 per day (if required, additional 8mg - $5.00)
 
Please note that different dose levels are different for different individuals and will be determined by our physician who will consider your input as appropriate.

Additional Charges
Extra Drug Screen - $12.00 (only required if positive on prior screen)
Oral Drug Screen - $5.00 (upon request)

Set-up Fee: $25.00
